Responsibilities:
Assist with resident assessment and implementation of care needs and will participate in resident and family care conference
Must be able to effectively communicate with residents, families, staff, physicians, outside providers and the public
Ensures that changes in resident condition are evaluated, monitored, documented and effective interventions are implemented
Provide support and assistance to medication management systems
Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored
Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management and pain management
Requirements:
Current licensure as a Practical Nurse ( LPN ) or Registered Nurse (RN) in the State of New Jersey
