POSITION SUMMARY
Assist adults with a mental illness and/or with a substance use condition to access needed treatment and services. Incorporate lived experience, knowledge of recovery principles and a trauma-informed, member-centered approach.
POSITION DUTIES
- Plan, conduct, and facilitate support groups, within program guidelines using established curricula and educational materials.
- Provide or assist in accessing services specified in the member service plans.
- Work individually with members and assist them in achieving the goals established in their Service Plans.
- Complete concurrent/collaborative documentation, including progress notes, screening notes, staffing notes and group notes.
- Participate in Adult Recovery Team Meetings and other clinical staffing in order to coordinate care with care managers and other clinical staff.
- Accompany clinical staff on home or community visits to members to provide outreach and/or wellness checks.
- Assist with coordinating care between clinical departments.
- Conduct member drug and/or alcohol screenings.
- Transport members to various activities, meetings or programs in support of treatment plan goals.
- Participate in agency job interviews for open positions.
- Performs other duties as assigned.
GENERAL EXPECTATIONS
- Provide high quality customer service to internal and external customers.
- Must work well under pressure, meeting multiple and sometimes conflicting deadlines.
- Demonstrate cooperative behavior with colleagues and supervisors.
- Participate in Quality Improvement and Performance Improvement Processes of the agency.
- Complete all TGC required trainings on time.
- Maintains a high level of professionalism while performing job functions.
- Demonstrate flexibility and a willingness to learn.
- This position is 100% onsite/TGC campus, in the state of Arizona.
EDUCATION/EXPERIENCE
Required:
- High school diploma or high school equivalency diploma.
- Previous or current involvement in behavioral health as a recipient of services.
- Minimum one year in recovery.
- Ability to complete Peer Support Certification within 180 days of hire (if not already certified).
- Current driver’s license and able to pass MVR screening.
- Possess skills in computer usage, active listening, reading comprehension, time management, customer service, speaking, monitoring, writing and learning strategies.
- Knowledge of community resources.
- Ability to cope with emotional demands as they arise and be able to secure appropriate support and help.
- Ability to remain calm when required to do several jobs at one time.
- Must be able to obtain and maintain an Arizona Level One Fingerprint Clearance Card.
- 21 years of age or older.
