COMMUNITY NAVIGATOR JOB DESCRIPTION

This position provides direct assistance to those experiencing or in danger of experiencing houselessness with the goal on ensuring stable housing. The position connects individuals in danger of or experiencing homelessness with resources necessary to end homelessness. There are three community navigators in Juneau, housed at AWARE, St. Vincent DePaul Society, and the Juneau Housing First Collaborative. All three positions are required to collaborate with one another to ensure efficient use of resources and to avoid service duplication. The position is expected to work on site at the shelters, as well as in the community, and will have regular hours at the Teal Street Center.

DUTIES AND RESPONSIBILITIES

· Assist with applications for housing assistance tenancy applications. Ensure that applications are complete, presentable, and give the potential resident the maximum chance of securing housing.

· Collaborate with the Bartlett Regional Hospital Case Management Team on discharge and other planning

· Assist individuals with moving into reentry program housing when exiting Lemon Creek Correctional Facility

· Develop and maintain relationships with landlords and landlord representatives to ensure that individuals in need of housing have access to housing units due to established contacts

· Assist with obtaining all documents necessary for getting and remaining in housing, such as IDs, income verification, and other relevant documents

· Assist with the physical process of moving people into housing (such as procuring furniture), and continue that support to ensure the best chance for long term housing success.

· Assist people with initial and ongoing rental subsidy related paperwork

· Assist people with remaining housed by mediating tenant/landlord issues and other disputes, as well as working with tenants on necessary modification in order to facilitate stable housing and prevent eviction.

· Act as an active link between those experiencing homelessness and the professional service agencies and healthcare providers in Juneau

  • Help with case management, the coordination of medical and social services care

NECESSARY QUALIFICATIONS

· Profound and deep felt respect and compassion for all people

· Appreciation of alternative life style

· Understanding of oppression, houselessness, substance abuse, and illness

· Ability to work independently and with others

· Bachelors Degree or an obvious proven track record of great work with populations experiencing homelessness and individuals experiencing tough circumstances

· Ability to utilize trauma informed approach

· Flexibility

· Appreciation for chaos

· Ability to multi-task

· Adequate technology skills

· Valid Driver’s License

OTHER

· This position is a part of the Community Navigator Team. There are several navigators, each placed in various social services agencies. This position is responsible to work with the other Community Navigators to ensure that services are delivered collaboratively throughout the community. All Navigators will meet weekly and will attend the Bartlett Regional Hospital Case Management meetings weekly to ensure that work is conducted collaboratively and resources utilized efficiently.

· The Navigator will work primarily out of the 8715 Teal Street and 1944 Allen Court office. Additionally, the position will travel with or without the patron to necessary locations and hold regular office hours at the Teal Street Center, 8711 Teal Street.

The Navigator will operate mainly during regular business hours but hours maybe alters during the winter to reach patrons in the Warming Shelter.
