Home Health Physical Therapist
Harrisburg, PA | Full-Time | Field-Based (Local Travel)

What You’ll Do
- Evaluate patients in their homes and develop individualized physical therapy care plans
- Deliver hands-on treatment and track patient progress toward functional goals
- Collaborate with nurses, therapists, and other clinicians as part of an interdisciplinary care team
- Educate patients and caregivers on home exercise programs, safety, and mobility
- Document visits accurately and on time using a mobile EMR platform
- Manage your own daily schedule and travel between patient homes
Schedule
- Monday through Friday, 8:00 am to 6:00 pm
- One weekend per month
- Two holidays per year
- Approximately 20% of your day spent driving to and from patient homes in the local area
What You Bring
Required:
- Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program
- Active Pennsylvania Physical Therapist license
- At least 1 year of healthcare experience
- At least 1 year of experience using an Electronic Medical Record (EMR) system
- Valid driver’s license and reliable transportation
- Comfort using tablets, smartphones, and EMR software
- Strong time management skills and the ability to self-manage a field schedule
- Commitment to meeting charting deadlines consistently
Preferred:
- Home health experience
