What are the responsibilities and job description for the Bilingual Community Health Worker position at The Children's Home Society of New Jersey?

The Community Health Worker will Perform a combination of community outreach and education, home visits, case management, group activities/workshops and community-based supportive services to provide a source of enhanced social support and create a bridge between under-served and hard-to-reach population and formal providers of health, social and other community services.

Flexible and Varied Work Schedule, Including Some Saturdays and Weekends - Depending on Programming

Work Location: Beachwood, New Jersey

Experience:

Minimum 1 -2 years of experience working with high-risk underserved, racially and ethnically diverse communities. Community outreach, grass root efforts, client engagement, and health medical background required. Maternal child health or work in a public health setting preferred. Verifiable good driving record and reliable transportation.

Bilingual English/Spanish (Written and Spoken) required.

Salary: $40,000.00/Annual

Education:

The qualified applicant must be bilingual (English/Spanish) with a High School diploma, Associates degree (preferred).

Responsibilities Include:

  • Conduct outreach with strong focus on high-need women not currently engaged in health care or other supportive community services, including homeless shelters, domestic violence shelters, methadone clinics, food pantries, soup kitchens, etc.
  • Expand outreach services in Monmouth and Ocean Counties, to women of childbearing age to recruit and retain prenatal and inter-conception clients and their families in a comprehensive system of care.
  • Collect data on all participants being served and work closely with Central Intake Referral Specialist(s) within the region to identify appropriate services.
  • Assist in promoting health insurance/Medicaid enrollment so that families in need are connected to these resources and case manage participants up to three (3) years from the time of enrollment or until the participant voluntarily terminates from the program. Case management will include but not limited to, providing ongoing follow-up and assessment of need. For women that are not eligible for home visiting programs or case management through Medicaid managed care, offer and provide case management services.
  • Provide community educational events monthly utilizing a variety of methods, activities or strategies to increase knowledge and awareness, facilitating health behavior change, improve prenatal outcomes and reduce infant mortality. Topics to include, but not be limited to risk reduction, infant health promotion, pregnancy and parenting, interconception care and other health prevention promotions.
  • Assist in the coordination and facilitation of quarterly consumer driven Advisory Board meetings. Twenty five percent of the board members must be consumers.
  • Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place. Submit monthly supplemental data sheet electronically to the state with review and approval of program supervisor.
  • Assist clients in accessing health related services, including but not limited to obtaining a medial home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.
  • Motivate women to be active, engaged participants in their health.
  • Effectively work with participants, staff, doctors, agencies, etc. from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.
  • Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representative, supervisors and office staff.
  • Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations.
  • Work closely with Central Intake and other Ocean/Monmouth partners to meet contractual obligations and provide information and data to Program Administrator as needed in preparation for community meetings and required reports on a monthly and quarterly basis.

Additional Responsibilities :

  • Mentoring a newly hired CHW upon their acceptance to the CLG training program as needed.
  • Offering peer support/guidance to newly hired CHWs.
  • Participating in preceptor training, if applicable on how to offer support to newly hire CHWs.
  • Other job duties as specified by NJDOH, such as contact tracing support, COVID-19 outreach and education, and other responsibilities as assigned
  • Additional reporting and documentation will be required after completion of mandated academic instruction to complete certification training as a CWH under the CLG CHW Institute.
