Lead Medical Coordinator

The Children's Community Programs of Connecticut
New Haven, CT Full Time
POSTED ON 5/20/2026
AVAILABLE BEFORE 6/18/2026

Children’s Community Programs is seeking a Lead Medical Coordinator (RN / Bilingual) to help ensure children in our Long- Term Foster Care program receive timely, coordinated, and compassionate health services. This is not a narrow scheduling role. It is a lead position for a bilingual licensed professional who can coordinate medical, dental, and public health services, support staff, strengthen follow-through, and help children in care access the services they need without being lost in the system.

This is the right opportunity for someone who wants meaningful impact, more schedule stability than traditional hospital work, and a role that blends coordination, leadership, and mission-driven service. You will experience:

  • More stability than hospital-based care. This full-time role is generally aligned to daytime hours, while still allowing the flexibility needed in child-serving work.
  • A leadership role with substance. You will oversee health-service coordination, support and guide staff, build staff knowledge through training, and help hold systems together across multiple touchpoints.
  • Visible mission impact. You will support children whose lives may include trauma, disrupted attachment, transition, and inconsistent access to services.
  • Office/community balance. The role is primarily office-based but includes some travel and real-world coordination with providers and community partners.

What you’ll do

  • Oversee and coordinate health services for children in care, ensuring timely and appropriate medical attention.
  • Supervise and support staff, providing guidance and oversight of daily responsibilities.
  • Develop and deliver staff training on medical, dental, and public health procedures and protocols.
  • Ensure availability and coordination of health interpretation and translation services.
  • Coordinate post-release healthcare services to support continuity of care.
  • Collaborate with healthcare providers and community resources to facilitate comprehensive care.
  • Monitor compliance with health regulations and organizational standards.
  • Maintain accurate and up-to-date medical records and documentation.
  • Participate in interdisciplinary team meetings to support holistic care planning.

What you’ll bring to the table

  • Licensed Registered Nurse (RN) with a bachelor’s or higher related qualification
  • Bilingual English and Spanish required (or English and Portuguese)
  • Minimum of 1 year of employment in public health or healthcare related environment with supervision preferred.
  • Demonstrated experience in care coordination, health administration, or direct clinical care.
  • Bilingual English and Spanish required.
  • Strong written and verbal communication, organization, time management, documentation discipline, and attention to detail.
  • A courteous, professional, culturally competent, trauma-informed approach to working with children and families.

Schedule, compensation, and work setting

  • Pay range: $85,000 – $90,000
  • Sign-on bonus: $3,000
  • Position type: Full-time
  • Schedule: Generally 8:30 a.m. to 4:30 p.m., with flexibility and varied hours if needed
  • Work environment: Primarily office-based with some travel
