What are the responsibilities and job description for the Primary Care LPN position at the Chickasaw Nation?
We are currently looking for a Full Time Primary Care LPN to join our Primary Care Clinic Team located at the Chickasaw Nation Medical Center.
Summary:
The LPN provides direct patient care and clinical support, including rooming patients, obtaining histories, administering medications and immunizations, assisting with procedures, and reinforcing patient education. LPNs collaborate closely with RNs, providers, and the interdisciplinary care team to facilitate communication, support care coordination, and ensure positive patient experiences and smooth clinic operations.
- Provides routine patient care by utilizing and documenting the nursing process: including teaching patients and their families
- Administers childhood and/or adult vaccinations according to facility policy, protocol, provider order and/or standing orders
- Provides age, population and culturally appropriate, holistic patient care based upon nursing department standards, policies and procedures
- Provides safe nursing interventions ensuring that it is appropriate to patients’ needs, age and condition
- Recognizes and anticipates common changes in the physical, biopsychosocial and health needs of all ages of patients experiencing urgent, emergent, or non-emergent conditions
- Rooms patients and conducts initial screenings in accordance with clinic workflows and facility policies
- Prepares patients for examinations, procedures, or tests and assists the provider with diagnostic, therapeutic, and surgical procedures and/or exams within scope of practice and in accordance with clinic workflows and facility policies
- Performs delegated clinical tasks, such as specimen collection, electrocardiograms (ECGs), dressing changes, suture removal, and wound care within scope of practice, per provider direction and facility policies
- Assists with preparing and facilitating laboratory, radiology, and medication-related orders in the electronic health record per provider direction and facility policies and procedures
- Sets up/cleans exam rooms and equipment per policy and restocks supplies in exam rooms
- Documents patient encounters and related activities, such as observations, phone communications, direct care, and nursing interventions, in accordance with facility policies and procedures
- Facilitates communication via telephone between the patient and the medical staff
- Protects the confidentiality, security, and integrity of information by adhering to applicable regulatory requirements and internal policies
- Participates in hospital, departmental, and CNDH performance improvement activities to measure, assess and improve the quality of care delivered to patients
- Maintains and promotes professionalism and is courteous to patients, staff, and visitors
- Exhibits prompt arrival to duty station as well as follows employee code book for time and attendance
- Participates in the orientation and onboarding of new staff and supports clinical education by facilitating shadowing experiences for nursing and other clinical students
- Attends 90% of clinic and outpatient department meetings or obtains minutes and assumes responsibility for the information given
- Contributes to the team in a positive way that promotes professionalism and collaboration between departments
- Function as a member of the multidisciplinary healthcare team by collaborating with CNDH providers and integrated service partners to deliver comprehensive, patient- and family-centered care
- Performs delegated clinical tasks from the RN within scope of practice and in accordance with facility policies and procedures
- Collaborates with the RN and interdisciplinary care team to support patient care and clinic operations
- Collaborates with the RN to provide patient and/or family education and management support designed to meet the age-specific needs of the patient population served
- Assists patients with obtaining and scheduling appointments and facilitates patient flow through the check-out process per clinic workflows and facility policies
- Seeks opportunities to assist other staff in the clinic without direction
- Provides first class customer service to both internal and external customers and responds to inquiries, compliments, and/or complaints with professionalism, courtesy, and efficiency
- Completes all training and development requirements and participates in continuing education opportunities
- Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types
- Maintains a clean, organized work area and ensures a safe environment

- Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics
