What are the responsibilities and job description for the Pediatric Clinic LPN Clinical Navigator position at the Chickasaw Nation?
We are currently looking for a Full Time Pediatric Clinic LPN Clinical Navigator to join our Pediatric Clinic Team located at the Chickasaw Nation Medical Center.
Summary:
Plans and provides routine patient care by utilizing and documenting the nursing process.
Responsibilities:
- Develops and maintains care planning for patients and reviews the care plan for assigned patients
- Provides and advocates age and culturally appropriate, holistic patient care based upon facility mission statements and as outlined in policies and procedures
- Calculates and safely administers all prescribed medications according to nursing standards and facility policy and protocols
- Administers vaccinations under the oversight of the registered nurse and according to facility policy
- Recognizes and anticipates common changes in the physical, biopsychosocial and health needs of patients experiencing urgent, emergent or non-emergent conditions
- Participates in initial screening of patient to include vital signs, weight, collects data on outside medications, chief complaint, and documentation of patients’ pain perception as appropriate to the visit Update wellness information performs patient history interviews, other appropriate sections, and documents in medical record
- Carries out patient history interviews and patient screens and documenting in medical record
- Assists provider with diagnostic, therapeutic, and surgical procedures, exams, specimen collection and other care as directed
- Implements interventions appropriate to the age-specific needs of the patient requiring outpatient care
- Collaborates with the RN to provide patient and/or family education designed to meet the age-specific needs of the patient population served
- Rooms patients and prepares patients for physical exams
- Facilitates lab and x-ray physician orders
- Sets up/cleans exam rooms and equipment and restocks supplies in exam rooms
- Removes sutures and changes dressings per provider order
- Notifies patient of lab results as directed by provider
- Documents observations, phone calls, referrals and interventions
- Facilitates the patient through the check-out process
- May serve as preceptor for nursing students
- Assists with coordinating referrals, i.e., purchased referred care, Medicaid, and private insurance
- Receives all patient consults and referrals and assists with determining prioritization in collaboration with Pediatric Clinic providers and interdisciplinary team
- Schedules patients in Pediatric Clinic with providers according to specific skill set and expertise
- Contacts patient for scheduling in accordance with the priority level
- Ensures patient orders and paperwork are completed for scheduling, referrals, and prior authorizations
- Coordinates any follow-up or special testing requests prior to the patient’s appointment
- Receives follow-up recommendations and results from patient procedures/referrals/consults and ensures that patient and patient’s care team is updated in a timely manner
- Manages appropriate provider schedules as directed by facility policy and clinic leadership
- Facilitates patient care by generating and faxing/copying patient protected health information necessary for continuation of care to outside healthcare facilities and/or providers, ensuring that all records needed for this process are accurate and valid, while maintaining patient confidentiality, security and integrity of data and information and noting within the patient’s EHR each disclosure
- Facilitates communication between the patient and the medical staff
- Facilitates consults, referrals, and form completion as requested and approved by the medical staff
- Provides first class customer service - courteous and helpful to all internal and external customer types
