Salary:
Starting at $23.00 an hour (Non-exempt).
About the Position:
Seek out and provide one on one contact with older adults or their caregivers to encourage use of existing services and benefits.
A Typical Day as a Senior Services Outreach Worker:
- Conduct search and find activities which seek out and identify hard to reach older persons and targeted populations.
- Conduct in-home visits and inform persons of benefits and services available to them.
- Encourage older persons to participate in senior programs.
- Assist older persons in gaining access to needed services such as making referrals for them.
- Conduct follow-up activities with older persons and or agencies to determine whether services have been received and the identified need has been met.
- Document all services in accordance with agency, state, and federal requirements.
- Conduct collateral contacts with individuals significant to assessed problem situation.
- Execute referrals to those community resources providing services which address relevant identified problem circumstances, with client's permission.
- Attend agency or inter-agency staffing to insure internal and inter-agency coordination of services.
- Assist with program promotion, presentation, or public education activities consistent with program and caseload responsibilities.
Work Hours:
Schedule is flexible and non-traditional; based on the availability of youth and families served.
Does the Following Apply to You?
- High School Diploma or GED
- Child Welfare Experience (preferred)
Additional Qualifications:
- Completion of DCFS criminal background check and fingerprinting
- Must be at least 21 years of age
- Obtain an insured vehicle
- At least three years of driving experience with a valid driver's license
