This position is responsible for assisting individuals with a designated behavioral health diagnosis or physical health condition in addressing their healthcare. The Care Coordinator will oversee and coordinate access to all of the services a member requires to optimize their health. This is a KPERS covered position.
Job Duties:
- Assist clients through the admission process, completing initial registration paperwork, completing assessments, gathering outcomes data, scheduling appointments and completing referrals to recommended services
- Provide care coordination services to eligible clients in amount and frequency appropriate to achieve goals specified in the client's treatment plan
- Communication and collaboration will occur across health care and social service systems as well as community resources and others identified by the client
- Accurately complete all required documentation including client registration and assessment paperwork
- Participate in agency training and ongoing professional development as required
- Attend treatment team agency required meetings.

Required Qualifications
- Background void of issues related to abuse or neglect
- Valid Kansas driver's license with favorable record
- Bachelors degree in psychology, social work, or related area; or 4 years of equivalent work experience
- Complete Targeted Case Management training prior to service delivery
Desired Qualifications
- Working knowledge of social service resources
- Knowledge of general recovery concepts
- Experience with program development and delivery
- Experience motivating/advocating for persons with disabilities
- Bilingual in Spanish

Pay: $24.04 - $26.44 per hour
