What are the responsibilities and job description for the Home Care Coordinator position at One and Only Home Care?
About the Role
We're seeking a detail-driven Home Care Coordinator to keep our clients' care running smoothly from intake through ongoing service. In this role you'll be the hub connecting clients, caregivers, families, payers, and our internal team making sure authorizations are in place, schedules are covered, and documentation stays clean and compliant.
The ideal candidate knows the home care world, understands how Medicaid and managed care work, and thrives on staying organized in a fast-moving environment.
What You'll Do
- Coordinate client care from intake and onboarding through ongoing service, ensuring timely follow-up at every step.
- Verify and track Medicaid eligibility, service authorizations, and reauthorizations; flag expiring or insufficient auths before they interrupt care.
- Serve as a primary point of contact for clients, caregivers, and families - answering questions and resolving concerns with professionalism and empathy.
- Coordinate with Managed Care Organizations (MCOs) and case managers on authorizations, plans of care, and service changes.
- Match and schedule caregivers to client needs, and manage schedule changes, call-outs, and coverage gaps.
- Maintain accurate, compliant client records and documentation in accordance with Medicaid, state, and payer requirements.
- Enter and manage visits, authorizations, and client/caregiver data in HHAeXchange (HHAX).
- Partner with clinical, compliance, and billing staff to keep documentation audit-ready and services properly authorized and billable.
What We're Looking For
Required
- 1 years of experience in home care, home health, or a similar in-home services setting.
- Working knowledge of Medicaid and the authorization/eligibility process.
- Strong organizational skills and the ability to juggle multiple priorities and deadlines without dropping details.
- Excellent interpersonal and communication skills, with a client-first, empathetic approach.
- Comfort working both independently and as part of a team in a fast-paced environment.
- High attention to detail and accuracy in records and documentation.
Preferred
- Experience working with MCOs / managed care contracts (authorizations, case manager coordination).
- Hands-on experience with HHAeXchange (HHAX).
- Familiarity with EVV, plans of care, and home care compliance requirements.
- Bilingual a plus.
