Job Description: The primary purpose of this position is to plan, organize, develop, and direct our facility's Physical Therapy Services in accordance with current applicable federal, state and local standards, guidelines and regulations, and as may be directed by the Administrator, to assure that the highest degree of quality resident care can be maintained at all times.

• Full time Physical Therapist will receive a $5000.00 sign on bonus.

Job Duties and Responsibilities:

• Assist in developing and maintaining physical therapy standards.
• Participate in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary.
• Maintain treatment records, resident files, and progress notes as required.
• Ensure business office receives charges for therapy services rendered to residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
• Assist in interviewing residents to evaluate daily living skills and capacities and determine extent of abilities and limitations.
Experience and Specific Requirements:

• Must have a Bachelor's degree in Physical Therapy from an accredited college or University.
• Must have a minimum of two years experience in a therapist capacity in a hospital, nursing care facility, or other related medical facility
• Must have training in rehabilitative and restorative therapy practices.
• Must have a current, unencumbered, active license to practice as a Physical Therapist in the state of Texas.
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
• Must be knowledgeable of physical therapy procedures.
• Able to plan, organize, develop, implement and interpret the programs, goals and objectives of the department as it relates to The Center at Foresight
