Schedule, Hours & Territory
- Dedicated Weekend On-Call RN Position
- Coverage Hours: Friday at 4:30 PM through Monday at 8:00 AM
- Territory: Cleveland, OH and surrounding service area
- Combination of scheduled patient visits and on-call responsibilities
- Primarily remote when not actively responding to patient needs in the field

Key Responsibilities
- Respond to after-hours and weekend patient, family, and caregiver needs by providing timely nursing assessments, interventions, and clinical support.
- Perform on-call visits to assess changes in patient condition, manage symptoms, and implement appropriate care plans.
- Coordinate care with physicians, caregivers, pharmacies, and interdisciplinary team members to ensure continuity of care.
- Provide education and guidance to patients, families, and caregivers regarding medications, symptom management, and end-of-life care.
- Facilitate admissions, deaths, and other urgent clinical situations in accordance with hospice policies and regulatory requirements.
- Complete accurate and timely documentation of patient assessments, interventions, and care coordination activities in the EMR.
- Deliver compassionate, patient-centered care while supporting the physical, emotional, and psychosocial needs of patients and families.
- Active Registered Nurse (RN) license in the state of practice required.
- Minimum of one (1) year of nursing experience required; hospice, home health, oncology, palliative care, or critical care experience preferred.
- Strong clinical assessment, symptom management, and critical thinking skills.
- Ability to work independently, prioritize urgent patient needs, and make sound clinical decisions in the field.
- Experience with electronic medical records (EMR); Homecare Homebase (HCHB) experience preferred.
- Excellent communication and interpersonal skills with the ability to effectively support patients, families, caregivers, and healthcare professionals.
- Valid driver's license, reliable transportation, and current automobile insurance required.
