KEY JOB RESPONSIBILITIES: Tasked with the responsibility of taking care of terminally ill patients at home. Operates in a systematic manner that takes care of the whole gamut of healthcare for terminally ill, death bound patients. Provides care that is well-tailored to the cultural, religious, and social needs of a dying patient.

Additionally, The Nurse Practitioner

  • Nurse Practitioner regularly assesses and reassesses the nursing needs of the Hospice patient.
  • APRN performs Face to Faces timely.
  • Nurse Practitioner provides dietary counseling.
  • APRN provides Hospice nursing services, treatments, and preventive procedures.
  • Nurse Practitioner initiates nursing procedures appropriate for the patients Hospice care and safety.
  • APRN observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patients physical or emotional condition.
  • Nurse Practitioner teaches, supervises, and counsels the Hospice patient and family members about providing care for the patient.
  • APRN supervises and trains other nursing service personnel.
  • Nurse Practitioner develops and re-evaluates the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care.
  • APRN performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctors orders.
  • Attends team conferences.
  • Maintains records as required by Hospice.
  • Nurse Practitioner follows the policies and procedures of Hospice. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
  • Always communicates to the supervisor if unable to meet a patients need or perform a procedure.
  • Nurse Practitioner participates in the on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
  • Maintain skills and knowledge.
  • Works with the interdisciplinary group concept of patient care.
  • APRN coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF, or MR.
  • Organizes work schedule and utilizes time management to be able to attend all required meetings.
  • Complies with agency infection control policies and protocols.
  • Assist with orientation, teaching, and training as requested.
  • The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patients home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.
  • Other duties as assigned by the Director.

Qualifications

  • One must be able to hear adequately on the telephone with no more than an amplifier and be able to communicate both verbally and in writing in English.
  • Maintains the agencys mission, philosophy, and core values.
  • Adheres to dress code, appearance is neat and clean, wears identification while on duty.
  • Reports to work on time and/or as scheduled.
  • Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
  • Ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.
  • Always maintains patient confidentiality, including all HIPAA regulations.

Regulatory Requirements

  • Licensed or registered in the State of Michigan.
  • Must pass a criminal background check & MVR check.
  • Completed health statements acknowledging the ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.
  • Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred.
  • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).

Industry Experience

  • Ability to work independently, make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crises outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes. Completes the Hospice training program.
  • Minimum 2 years of experience as a registered nurse, preferred.
  • Active patient contact within the past three years is preferred.

Education

  • Graduate from an accredited school of nursing.
  • Currently licensed as a Registered Nurse and Nurse Practitioner to practice in the state.
