Key Responsibilities:
- Lead and support daily clinical operations while ensuring compliance with hospice regulations, company policies, and quality standards.
- Mentor, educate, and develop clinical staff through coaching, orientation, documentation review, and ongoing performance support.
- Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary team collaboration to ensure exceptional patient outcomes.
- Monitor quality metrics, conduct chart audits, and support survey readiness and performance improvement initiatives.
- Partner with branch leadership, physicians, referral sources, and operational teams to promote clinical excellence and outstanding patient and family experiences.
- Active Registered Nurse (RN) license required.
- Minimum of two years of RN experience, including at least one year of leadership experience in hospice, home health, or a related healthcare setting.
- Experience with Homecare Homebase (HCHB) preferred.
- Strong leadership, clinical assessment, communication, and organizational skills.
- Ability to coach teams, manage multiple priorities, and thrive in a fast-paced healthcare environment.
- Valid driver's license, reliable transportation, and current automobile insurance required.
