Schedule, Hours & Territory
- Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM

- Provide personal care, hygiene, and comfort measures
- Observe and report changes in patient condition
- Support patients and families with compassion and respect
- Document care provided accurately and timely in the EMR HCHB (Homecare Homebase)
- Active CNA certification
- Hospice aide experience preferred
- Experience using HCHB (Homecare Homebase) preferred
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
