Hospice Clinical Manager (RN)

Join The Care Team as a Clinical Manager and lead a compassionate team dedicated to providing exceptional end-of-life care to patients and families. We are seeking an experienced Registered Nurse leader to support clinical operations, mentor and guide staff, ensure high-quality patient care, and help drive excellence across our hospice team.
Schedule & Coverage
• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• This role requires you to work in the office five days per week at our Bethlehem, PA, location.
• This role may require you to complete RN visits in the field if RN Case Manager caseloads exceed our standard range and provide additional field support as needed.

Key Responsibilities:
• Lead and support daily clinical operations while ensuring compliance with hospice regulations, company policies, and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, documentation review, and ongoing performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary team collaboration to ensure exceptional patient outcomes.
• Monitor quality metrics, conduct chart audits, and support survey readiness and performance improvement initiatives.
• Partner with branch leadership, physicians, referral sources, and operational teams to promote clinical excellence and outstanding patient and family experiences.
Qualifications:
• Active Registered Nurse (RN) license required.
• Minimum of two years of RN experience, including at least one year of leadership experience in a hospice setting
• Experience with Homecare Homebase (HCHB) preferred.
• Strong leadership, clinical assessment, communication, and organizational skills.
• Ability to coach teams, manage multiple priorities, and thrive in a fast-paced healthcare environment.
• Valid driver's license, reliable transportation, and current automobile insurance required.
