JOB SUMMARY:
The Nutrition Site Manager is responsible for provision of nutrition services to participants documentation of services, development of lesson plans and participants' care plans. In addition the Nutrition Site Manager is responsible for the implementation of the annual nutrition education/ outreach objectives, as well as training ano supervisinq all exempt staff at the site.
DUTIES AND RESPONSIBILITIES:
Plans, directs, implements and evaluates the Nutrition Education component of the NYWIC Program, Trains, supervises and evaluates NYWIC supervisory, coordinator staff at site Responsible for monthly inventory of checks and monthly reconciliation of checks at the sites, includind check distribution and disposition Performs job evaluations, time card and oversee entire program functions at the site. Conducts auality assurance review on all NYWIC staff involved in the check issuance process as well as anthropometry and hematology process. Maintains documentation on file at site Conducts monthly participant surveys and monitor VOC's out of site. Completes quarterly self-assessments in compliance with NYWIC Program policies and procedures at sites and send report to the NYWIC Director, Maintain the following documents at the sites in a current and updated format:
(1) State NYWIC Program manual which is accessible to all NYWIC Staff.
2) An approved Nutrition services management plan which is accessible to NYWIC Local agency, State and USDA staff.
3) A staff training log for all staff indicating Agenda. dates and attendees.
4) A local agency NYWIC policy and procedure manual. 5) NYWIC loca agency memos and NYWIC Communiqué log.
NYSDOH memos and reports as distributed Prepares confidential records and cooperate fully with State Investigative Services staff. as well as other authorized persons, agencies and entities identified in NYS NYWIC contracts. Assists the Director to build department cohesion and helps staff develop confidence and a positive customer service attitude Assists Program Director in writing and implementing policies and procedures for service delivery, especially those that involve the Nutrition Education component Manages the day-to-day operations of the NYWIC Program and establishes mechanisms designed to ensure the program's integrity and maintain/exceec assigned caseload Subject to budgetary considerations, assesses the need, orders and maintains supply of office supplies, forms and nutrition educational materials to support the Nutrition Education component of the NYWIC Program. Evaluates and recommends the purchase of educational materials that are based on sound scientific principles and meet the literacy skills and cultural diversity of the NYWIC population served Assesses and certifies NYWIC participants Develops educational/care plan for low and high risk participants. Educates and documents contact for these participants individually and in group settings Assigns age appropriate food package using the check matrix., Develops and implements participant centered nutrition education using appropriate handouts Assists the NYWIC Director in developing some of nutrition section of the Nutrition Services Plan/ LACASA. Implements procedures, monitors and evaluates results Maintains adequate program, employee and client records. Maintains statistical information for evaluation and planning of nutritional services Assists the Program Director in preparing progress reports, grants, proposals and annual evaluations and plans as required Immediately report any problems or unusuak occurrences to Program Director. Keeps abreast of new developments in nutrition Ensures that the NYWIC Proaram adheres to al mandates and regulations of the Program, including the contracted caseload. annual implementation of the Farmer's Market Program. Ensures staff is trainec in policies and procedures of the Program. Performs other related duties as assigned.
KNOWLEDGE SKILLS AND ABILITIES
Excellent communications, public relations planning, analysis, organizing, writing, group presentations individual counseling and working with diverse group of people. Bilinqual - Spanish, Mandarin and/or Cantonese Data entry and computer literate NYWIC competencies.
EDUCATION: Bachelor's or Master's Degree in Nutritional Science Community Nutrition, clinical or Human Nutrition, Dietetics or Foods and Nutrition or Public Health Nutrition, from a regionally accredited college or universitv or one recoanized by the New York State Education Department Master's Dearee in Public Health - Preferred WORK EXPERIENCES: Four (4) years experience as a Nutritionist in a clinica or community setting, of which at least two (2) must be in a NYWIC Clinic. Supervisory experience highly desirable. CERTIFICATION: Registered Dietitian (RD), Certified Dietitian Nutritionist (CDN) Highly desirable, but not required,
