Resource Advocate/Care Manager Extender for Tailored Care Management
As a Resource Advocate in our Tailored Care Management program, you will play a key role in providing whole-person care to individuals with Down Syndrome, Autism, Cerebral Palsy, and other intellectual/developmental disabilities. You will work closely with the Care Managers to help individuals access the resources and support they need. Your responsibilities will include contacting individuals to build relationships, coordinating appointments, making referrals, and connecting them with necessary community resources. In addition to these tasks, you will provide education, information and advocacy to support individuals to reach their goals.
To be successful in this role, you must be highly organized, possess excellent communication skills, and demonstrate a strong commitment to helping others. You should be able to navigate the service systems effectively and work collaboratively with individuals to ensure they receive the support they need. Position will primarily support individuals living in Durham and Orange Counties.

Minimum Requirements:
18 years of age with a high school diploma or GED
Two years of experience working directly with individuals with intellectual and/or developmental disabilities OR
Be a person with lived experience of I/DD OR
Be a parent or guardian of an individual with an I/DD with at least two years of direct experience providing care for and navigating the system on behalf of that individual
