The Arc of Greater Haverhill- Newburyport is a grassroots 501(c)(3) nonprofit founded in 1962 to advocate with and for individuals with intellectual and developmental disabilities and their families throughout the Merrimack Valley and seacoast communities.
We are currently seeking a full-time RN in our Adult Family Care (AFC) program to oversee a caseload of individuals with intellectual and/or physical disabilities or frail elders in a family-based setting. This position is starting at 40 hours per week, Monday through Friday. The Lead RN will work in conjunction with the AFC Coordinators as part of a multidisciplinary team in order to provide supervision, training, and case management via home visits.

RN Key Responsibilities:
- Conduct initial and ongoing assessments of client’s health status, safety in the community, potential resources needed, evaluate and maintain program eligibility, and complete bi-annual health assessments
- Collaborate with multi-disciplinary team to develop and implement care plans with focus on health maintenance and preventative care
- Train, support, and supervise caregivers in partnership with the care coordinator
- Oversight of medical needs and follow up care/Communication with healthcare providers
- Conduct monthly/bimonthly home visits and complete required documentation
QUALIFICATIONS:
- Valid Massachusetts RN License required;
- 2 years of recent experience in the direct care of elders or adults with disabilities required;
- Prior community based nursing experience preferred;
- Experience with MDS nursing assessment tool preferred.
- Fluent in Spanish highly desirable;
- Excellent written and communication skills;
- Valid driver's license and reliable transportation;
- Satisfactory background check and driving record;
- Computer Skills including experience with Microsoft office is required.
Full-Time Schedule: 40 hours per week Monday through Friday. Availability to work some evenings until 5:30 as needed, with a flexible schedule.
