The **Arc of Cumberland and Perry Counties (CPARC)** is seeking a **full-time Program Specialist** to work at the **Life Time Adult Day Care center in Lemoyne.**
The **Program Specialist** is responsible for the development, implementation, and documentation of **Individual Support Plans (ISPs)**. This role also serves as an **advocate for participants**, ensuring they receive necessary services and maximum benefits from the program. The Program Specialist is primarily responsible for monitoring the individuals who attend Life Time under **2380 regulations**.
Key Responsibilities
The **Program Specialist** is responsible for the development, implementation, and documentation of **Individual Support Plans (ISPs)**. This role also serves as an **advocate for participants**, ensuring they receive necessary services and maximum benefits from the program. The Program Specialist is primarily responsible for monitoring the individuals who attend Life Time under **2380 regulations**.
Key Responsibilities
- Provide direct care and personal assistance to program participants as needed.
- Manage and coordinate services for an assigned caseload (up to 30 individuals).
- Lead and participate in ISP development, reviews, assessments, and updates.
- Document individual progress and participation on a monthly basis.
- Review ISPs with participants every 3 months and share updates with the support team.
- Ensure compliance with regulations, agency policies, and documentation requirements.
- Collaborate with the Director and instructors to support Community Participation activities that reflect individual interests and goals.
- Train direct service workers in health and safety needs relevant to each individual.
- Maintain accurate files, records, and safety protocols.
- **Master's degree** in a related field 1 year experience working directly with individuals with disabilities, **OR**
- **Bachelor's degree** in a related field 2 years experience, **OR**
- **Associate's degree/60 credits** 4 years experience.
- Strong organizational skills, ability to advocate effectively, and knowledge of regulatory requirements.
- **Starting Pay: $17.17/hour**
