Developmental Health Services (DHS) Treatment Coordinator
Full-Time | Elmira, NY | $25-$27 per hour

Join our team in Elmira, NY as a DHS Treatment Coordinator, where you'll play a key role in supporting individuals through coordinated care, treatment planning, and collaboration with multidisciplinary teams.
In this role, you'll lead the coordination of clinical services, treatment plans, and intake processes, ensuring individuals receive high-quality, person-centered care. You'll work closely with Medical Directors, Care Managers, families, and service providers to deliver effective and compliant services.
Key Responsibilities:
Coordinate clinical treatment plans, services, and follow-up care
Conduct intakes, admissions, and obtain required authorizations
Serve as liaison between providers, Care Managers, and families
Supervise and support assigned staff
Ensure accurate documentation, audits, and HIPAA compliance
Qualifications:
- Bachelor's degree in Psychology, Social Work, or related field
- At least 1 year of experience in clinical intake, case management, or healthcare coordination
- Strong communication, organization, and documentation skills
Preferred:
Experience with Microsoft Office (Excel, Word, Outlook, PowerPoint)
Familiarity with electronic health records (EHR) and clinical documentation
