Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patients’ health by taking vital signs and observing for any changes in condition or behavior
- Prepare nutritious meals and assist with meal planning to meet individual dietary needs
- Administer medications following care plans and ensure proper documentation in compliance with HIPAA regulations
- Support patients during outings or community activities to promote social engagement and independence
- Maintain a clean and safe environment through regular cleaning tasks and infection control practices
- Document care activities accurately and communicate effectively with healthcare teams to update on patient progress or concerns
Skills
- Proven experience in home & community care, assisted living, nursing homes, or group homes with a focus on senior or disability care
- Knowledge of patient observation techniques, medication administration, and vital signs monitoring
- Familiarity with specialized care such as Alzheimer’s/dementia care, memory care, autism support, hospice care, and working with individuals with developmental disabilities or disabilities
- Ability to perform heavy lifting safely using equipment like Hoyer lifts and assist with transfers
- Strong understanding of HIPAA compliance to protect patient privacy and confidentiality
- Excellent caregiving skills including assisting with ADLs, meal preparation, and creating personalized care plans
- CPR and First Aid certification to respond effectively during emergencies
- Compassionate attitude combined with excellent communication skills for working closely with clients’ families and healthcare teams
