Process incoming medical record requests from patients, healthcare providers, insurance companies, third parties, and other authorized requestors in accordance with organizational policies and applicable regulations.
Review requests for completeness and validate authorizations to ensure compliance with HIPAA and other privacy requirements prior to release.
Process requests from legal entities, including attorneys, subpoenas, court orders, and other legal documentation, following established procedures and timelines.
Prepare, compile, and release medical records in an accurate, timely, and secure manner.
Document, track, and maintain records of all disclosures in accordance with organizational and regulatory requirements.
Protect the confidentiality and security of protected health information (PHI) at all times.
Communicate effectively with patients, staff, attorneys, and external agencies regarding medical record requests and release procedures.
Prioritize and manage multiple requests while meeting established deadlines and service standards.
Utilize electronic health record (EHR) and document management systems to locate, retrieve, and release information.
Assist with filing, scanning, indexing, and maintaining medical records to ensure accuracy and accessibility.
