Seeking a Discharge Planner to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Priest Drive Health Center.
Full-Time Employed: 40 hours/week
M-F 8:30a - 5:30p
Location: 1642 S Priest Dr building 6 suite 101, Tempe, AZ 85281

Duties may include:
- Activities will be performed in both inpatient and outpatient settings.
- Connect individuals receiving Level I inpatient services with ongoing TERROS Outpatient treatment upon discharge.
- Work with BHRs while in the hospital, will work with transitioning BHRs, the hospital unit staff, the Outpatient team, family, RBHA care management, PCPs and Health Plans as clinically indicated to ensure effective transition to Outpatient or community services.
- Reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and TERROS psychiatric provider.

- GED/High school diploma with a minimum of 4 years of behavioral health employment experience, or
- An associates degree in the field of behavioral health and 2 years of behavioral health employment experience, or
- A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health employment experience, or
- A bachelor's degree in the field of behavioral health, or
- A master's degree or higher in the field of behavioral health
- Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred
- Valid AZ Driver License. Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy.
- Must have a valid unrestricted Arizona Fingerprint Clearance Card (Level One) or apply for an Arizona Fingerprint Clearance Card within 7 working days of employment with no disqualifying events.
- Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
- Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
- Must pass a typing test with a minimum speed of 25 WPM.
- Must pass background check, TB test and other pre-employment screening.
