What are the responsibilities and job description for the Adult Foster Care Supervisor position at Tempus Unlimited, Inc.?
The Adult Foster Care (AFC) Supervisor provides support, training and coaching to AFC caregivers and program members. They play a key role in ensuring quality care by offering support to Care Managers, conducting site visits, and providing oversight for the Community Services Adult Foster Care program. The AFC program itself offers a home-based care setting for individuals who need assistance with daily activities.
Essential Functions
Provide direct support to the care managers/nurses in the community
Conduct site visits to consumer homes and produce proper quality feedback /reports according to manager’s guidance
Offer input to annual performance evaluations
Provide monitoring and oversight for efficiencies with all referrals in AFC
Lead and assist with program team meeting
Support and train staff during the orientation processes or initial planning meeting to insure proper efficiencies in services
Support Community Living Program Manager as assigned
Monitor LTSS process to ensure accuracy and timely submittals
Assign caseloads
Provide supervision to nurses and care managers
Handle member or caregiver complaints
Support new program development and implementation with program
Additional Responsibilities As Assigned
Competencies
Three or more years of recent experience working with people who are elderly or disabled
Ability to supervise and direct staff
Excellent organizational skills
Attention to accuracy and detail
Strong interpersonal skills – telephone skills
Excellent oral and written communication
Good computer and analytical skills
Innovative thinking
Ability to maintain confidentiality
Preferred Experience
Perform all other duties as may be required
Supervisory Experience Preferred
Required Education
Bachelor's degree required
A Human Services degree is preferred but can be substituted with a related degree with field experience
Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines
