What are the responsibilities and job description for the Supervisor, Care Management - 40/hrs Day Shifts (Temple Hospital Jeanes Campus) position at Temple Health – Temple University Health System?
The Supervisor of Care Management serves in a leadership role and supports the Director in daily leadership responsibilities. In collaboration with the patient/family, physicians, and the interdisciplinary team, the Supervisor of Care Management assists the Case Manager/Social Worker in facilitating the patient's progress through the acute episode of care in an efficient and cost effective manner. Monitors the level of care, initiating evidence based protocols and order sets according to the patients' admission diagnosis, influencing the progression of care, facilitating the patients' navigation through the acute care episode, and proposes an appropriate transition plan within 48 hours of admission. The Supervisor of Care Management serves as an advocate for the Care Management Department and the staff that is overseen. Case Manager certification required within 1 year from date of hire.
Education
Bachelor's Degree BSN Preferred
Other Graduate of an accredited school of nursing Required
Experience
2 years experience in a Hospital setting as a Case Manager and/or 2 years experience managing a case management department Required
Licenses
PA Registered Nurse License Required
Certified Case Manager Required
Multi State Compact RN License Required Or
Education
Bachelor's Degree BSN Preferred
