Role Summary
The Licensed Vocational Nurse (LVN) Care Coordinator plays a vital role in supporting patient care coordination, continuity of services, and quality improvement initiatives within a value-based care environment. Duties
Conduct telephonic outreach to patients attributed to value-based care contracts.
Identify and address quality care gaps to support improved patient outcomes.
Participate in quality improvement programs and initiatives as assigned.
Support Primary Care Providers by assisting patients with scheduling preventive screenings and appropriate medical appointments.
Maintain consistent communication with healthcare providers through meetings, reporting tools, and coordination platforms.
Monitor value-based care quality performance and generate reports to identify open care gaps.
Perform outreach on behalf of providers to close care gaps and support medication adherence, contributing to quality performance and rating outcomes.
Provide patient and family counseling and health education using standardized protocols and approved educational materials.
Serve as a subject matter expert in care transitions and quality metrics.
Support continuity of care initiatives across diverse patient populations.
Meet expectations aligned with professional competency standards, including leadership, accountability, and collaboration.
Other duties as assigned.
Required Qualifications
Active Licensed Vocational Nurse (LVN/LPN) license.
Must be licensed in and reside in the state of Texas.
Minimum of 3 years of clinical or home health experience.
High School Diploma required (proof of education must be submitted).
Strong understanding of care coordination, quality improvement, and patient engagement within value-based care models.
Excellent communication skills, particularly in telephonic patient outreach and provider collaboration.
Desired Qualifications
Experience working with high-risk or multi-morbidity patient populations.
Familiarity with quality metrics, care gap closure, and preventive care initiatives.
Prior experience supporting primary care practices, ACOs, or clinically integrated networks.
Strong organizational skills and comfort working with reports and performance data.
