DutiesProvide personal care and escort assistance based on each patient’s individualized care plan, including grooming, dressing, bathing, toileting, peri-care, housekeeping, meal preparation/feeding, laundry, and related tasks.Observe and report changes in a patient’s condition to the team to support coordinated and effective care.Demonstrate safe practices by using proper body mechanics, following universal precautions, and maintaining a safe environment.Interact with patients, families, and the team in a positive, professional, and supportive manner; encourage engagement in conversations and activities.Perform additional related duties as assigned. Required QualificationsMinimum 1 year of experience working with the elderly or frail population.Ability to work both independently and collaboratively as part of a team.Flexibility to work in different care environments, including in-home or center-based settings.A positive, compassionate, and conscientious attitude with a strong commitment to patient-centered care.Basic conversational English skills; testing may be required.Bilingual skills may be required depending on patient needs; language testing may be required.Ability to translate or interpret basic information for supervisors, patients, and fellow caregivers (testing may be required).Ability to document care-related information in basic English using designated forms or electronic health records.
