Tedford is seeking a full-time (32 hours) Clinical Coordinator to assist with compliance and quality assurance of our TCM services delivered by our Case Management team and to support shelter staff in meeting the complex mental health and behavioral needs of guests. The Clinical Coordinator will also carry a small caseload of Case Management clients as needed.

For over 35 years, Tedford Housing has been empowering people to move from homelessness to home. Based in Brunswick and serving Sagadahoc, Lincoln, Cumberland and Androscoggin counties, Tedford has relocated its shelter programs and administration to a newly expanded facility. The individual in this role will work 12:00 pm to 8:00 pm 4 days a week.

This is a 32-hour a week, benefitted position, with a pay rate of $35 per hour.

The Clinical Coordinator is responsible for overseeing the compliance with MaineCare funded TCM services including:

  • Reviewing ISPs (Individual Service Plans) and providing signatures for all ISPs.
  • Ensuring the provision of quality case review of client records, including the ISP per MaineCare regulations
  • Provide and document monthly supervision to all Case Managers.

This position will carry a small caseload of clients to provide direct case management as needed. Case management tasks include:

  • Enroll assigned clients into Emergency Services & Housing Assistance Program (ESHAP) and/or Targeted Case Management (TCM) including data entry into the Homeless Information Management System (HMIS)
  • Ensure sound household intake and treatment and strength-based planning leading to the establishment of goals reflecting housing first principles and achieving stability. 
  • Complete comprehensive and coordinated entry assessments and work with clients to develop a person-centered Plan of Care (POC) and engage with guests/clients to establish action steps. 
  • Connect clients with community resources and assist clients with obtaining necessary documentation to overcome barriers and secure permanent housing. 
  • Support client transition plans and goals through discussion and problem solving. 
  • Assist clients with their housing search through securing appropriate housing vouchers and referrals to area landlords/property managers. 

Additionally, the Clinical Coordinator will also be expected to provide clinically-focused supervision and training to other agency support staff including:

  • Participation in regular Support Staff meetings
  • Participation in monthly Case Management team meetings
  • Participation in the training of new staff as well as ongoing training for support staff as needed
  • Providing clinical guidance to shelter team to ensure guests are met with trauma informed, clinically appropriate support
  • Providing training, ongoing clinical guidance and support to warming center staff during winter season including assisting with an intensive on boarding for all warming center staff.
  • Participation in the rotation for weekend on call shifts to provide administrative support and guidance to shelter staff. These on call shifts would occur 1-2 weekends a month.

The Clinical Coordinator will also play an integral role in responding to escalations both in shelter and at the warming center during work hours. The Clinical Coordinator should be familiar with local crisis services and will support guests in connecting with these services as appropriate.

The Clinical Coordinator must have one of the following credentials:

  • Licensed Clinical Social Worker (LCSW)
  • Licensed Clinical Professional Counselor (LCPC)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Master of Social Work (LMSW)

Other Qualifications:

  • Experience working with people with mental illness and substance abuse challenges
  • Familiarity with local and statewide mental health services, including Crisis services
  • Familiarity with local housing, medical, and other supportive services
  • Ability to use a trauma informed approach to verbally de-escalate tense situations
  • Basic knowledge of homelessness and its causes
  • Able to make a commitment to the Tedford mission and organizational values
  • Ability to motivate others and assist with positive change
  • Good written and oral communication skills
  • Ability to work independently
  • Good organization skills and ability to multi-task
  • Valid driver’s license, good driving record and reliable transportation
