Job Title: Care Manager II
Location (On-site, Remote, or Hybrid?): Newtown Square, PA (remote)
Contract Duration: Contract until 08/31/2025
Work Hours: Monday-Friday 8:30am-5pm
Duties:
Under the Supervision of the Manager of Care Management, the Care Manager is responsible for the overall accountability for the identification of members appropriate for care management services.
- Completes comprehensive assessments and gathers pertinent information about the members’ needs by interviewing the members, appropriate family members, physicians and other members of the members Care Team to develop, monitor and evaluate the member’s Individualized Care Plan and update as needed.
- Presents members in ICT rounds providing updates on members’ current health status, questions or concerns.
- Serves as an advocate for the member and acts as a liaison between the member and other community-based agencies, facilities, providers, and practitioners in coordinating the member’s care.
- Supports care post transition of care for members received from transition of care team.
- Monitors appropriate utilization of the member’s benefits and coordinates services with other payer sources.
- Perform all other duties and projects assigned.
Skills: Minimum of one year care management experience
