Community of Hope is seeking a Cervical Cancer Prevention Navigator to support clinical processes that promote the early detection and prevention of cervical cancer. This role ensures timely cervical cancer screening by reminding providers to order appropriate tests and obtaining records from external providers to verify that patients are up to date with recommended screenings. The Navigator also provides patient education, care coordination, and ongoing support for individuals with abnormal screening results or a cervical cancer diagnosis.

What You'll Do

  • Facilitates and schedules patient appointments/referrals with appropriate provider(s) for cervical cancer screening, diagnostics, and treatment. Attends appointments with patients on an as needed basis.
  • Ensures that COH patients complete cervical cancer screens or have recently had screens done and the records have been received by COH. Reviews patient records and flags providers to ensure that screening is complete.
  • Makes confirmation calls/emails to patients regarding future appointment dates and assists patients in arriving to scheduled appointments on time and prepared.
  • Educates patients on care processes and provides counsel to reduce anxiety and fear.
  • Identifies patient barriers to care such as transportation, housing instability, insurance, and language barriers, and refers patients as needed to additional resources at COH and other partner organizations.
  • Educates patients on the Human Papillomavirus vaccine as a method of cervical cancer prevention.
  • Supports clinic quality improvement (QI) efforts regarding cervical cancer screening, prevention, and referrals. This includes directing Plan-Do-Study-Act (PDSA) change cycles.
  • Using Electronic Health Record (EHR) system, tracks navigation services, records messages from patients, and contributes to clinic tracking workflows; including obtaining results from external facilities.
  • Tracks appropriate patient-level quality outcomes for program monitoring and evaluation.
  • Completes other tasks as assigned by supervisor.

Requirements

Must-Haves

  • Associate or Bachelor’s degree or equivalent experience.
  • Valid BLS Certificate.
  • 1-2 years of experience with health care clinic operations or at a community-based organization; preferably a Federally Qualified Health Center.
  • Ability to work collaboratively with clinicians, internal staff, and external organizations.
  • Commitment to addressing health disparities and barriers posed by social determinants of health.
  • Excellent time management and coordination skills, with the ability to set priorities given multiple work assignments.
  • High degree of independence, flexibility, initiative, and commitment.
  • Strong communication skills with diverse populations.
  • Proficiency with Microsoft Word and Excel.
  • Experience with an Electronic Health Record.
  • May be required to travel to other sites as needed.
  • Proof of required vaccinations is required. This includes, but may not be limited to, Flu. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.

  • Prior experience in public health or human services and/or community clinic or similar role.
