Community of Hope is seeking a Perinatal Case Manager, HONEY to assess patient needs during the pregnancy and postpartum period, developing individualized care plans and ensuring patients have timely access to healthcare and educational services that will remove barriers to care and improve overall outcomes for parent and baby. This is a full-time, temporary position located at our Family Health & Birth Center in NE, Washington, DC

What You'll Do

  • Ensures access to supportive and educational services for prenatal and postnatal patients with an emphasis on participating in prenatal and postpartum healthcare (whether at Community of Hope or another prenatal provider), increasing understanding of maternal and child health needs, coordinating social services and parenting resources.
  • Completes biweekly home and community visits to engage families in their natural environment, assess needs, monitor progress toward goals, and ensure interventions are responsive to each family’s home environment.
  • Maintains a caseload of about 30 participants who are referred from DC-based housing providers, medical providers, or the Virginia Williams Family Resource Center, providing care coordination services to ensure access to perinatal health and early child health services.
  • Conducts thorough assessment on all prenatal/postpartum clients utilizing Healthy Screening tools (HITS, 4P’s, PHQ-9, Edinburgh). Follows up and refers patient appropriately based on risk.
  • Utilizes non-COH data sources, including CRISP, to coordinate care for high-risk clients to ensure providers have access to accurate and complete medical information at the point of care; and ensure clients have a smooth transition of care from emergency department visits.
  • Develops care plans with designated periods with particular emphasis on risk factors, including but not limited to social needs, barriers to care, pregnancy planning, and food insecurity.
  • Ensures all prenatal and postpartum clients utilize referrals provided through weekly reminder calls. Assists with provider appointments, including but not limited to, following up/rescheduling of missed appointments, and ensuring access to care.
  • Assists as a liaison between the client and their families, homeless services provider, clinical staff and other health care professionals.
  • Documents any relevant resource connections and/or referrals, client encounters, including face-to-face, telephone, and electronic communications, in eCW within 2 business days and other systems per HIPAA and departmental policies and procedures.
  • Completes other tasks as requested by supervisor.

Requirements

Must-Haves

  • High school diploma, G.E.D, or equivalent experience.
  • Valid BLS Certificate.
  • Experience providing social services, care coordination or health care education.
  • Familiarity with perinatal services and standard of care.
  • Strong organizational skills. Ability to multi-task and problem-solve.
  • Strong written and verbal communication skills.
  • Experience conducting home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance.
  • Ability to travel between sites required.
  • Willingness and ability to make home visits and hospital visits, and work occasional evenings and weekends.
  • Proof of required vaccinations is required. This includes, but may not be limited to, Flu and Covid. COH will consider requests for reasonable accommodations for anyone who cannot be vaccinated for a religious or medical reason, subject to applicable law.

Nice-to-Haves

  • B.A./B.S. or equivalent experience required; B.A./B.S. in social work, education, or related field.
  • Experience working with an under-resource population, including pregnant people.
  • Familiarity with the Medical Assistance system and community resources strongly.
