The Certified Peer Support (CPS) is an integral part of the ACT Team. The CPS is responsible for carrying out rehabilitation and support functions and assist in treatment; education, support, and consultation to families; psychoeducation; and crisis intervention.. The CPS coordinates and provides services and resources to adult clients necessary to promote recovery, rehabilitation and resiliency. The CPS is accountable to and receives clinical supervision from the ACT Team Lead, Clinical Director, and Psychiatrist. The CPS works in collaboration with all staff and treatment team members. The CPS is responsible for working in partnership with clients to conduct intake, strengths-needs assessment, triage, safety planning, care coordination, in-home skills therapeutic activities, crisis intervention services, and Plan of Care development/implementation/evaluation.
