Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our dynamic team. The ideal candidate will possess a strong foundation in physiotherapy techniques and a passion for improving patient outcomes through personalized care plans. As a Physical Therapist, you will evaluate patients, develop tailored treatment strategies, and deliver evidence-based therapies across inpatient, outpatient, home health, and community care environments.
Responsibilities
- Assess patients’ physical conditions by reviewing medical histories and conducting thorough examinations.
- Develop comprehensive care plans that address individual needs, including discharge planning and rehabilitation goals.
- Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote recovery.
- Collaborate with multidisciplinary teams to coordinate inpatient, outpatient, and home & community care services.
- Monitor patient progress regularly and adjust treatment plans accordingly to optimize outcomes.
- Document all patient interactions accurately within electronic medical record (EMR) systems in compliance with healthcare regulations.
- Educate patients and their families on physiotherapy techniques, injury prevention, and self-care strategies.
Skills
- Extensive knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
- Proven experience in discharge planning, acute care, orthopedics, geriatrics, pediatrics, and home health settings.
- Strong understanding of care plan development for diverse populations across inpatient and outpatient environments.
- Proficiency with EMR systems for documentation and patient management.
- Excellent communication skills to effectively educate patients and collaborate with healthcare teams.
- Familiarity with hospital experience, physiotherapy modalities, trigger point therapy, and outpatient clinic procedures.

Pay: $57.00 - $60.00 per hour
