Direct Care Provider

Job Summary
TEAM Habilitation and Respite is seeking compassionate, dependable, and motivated Direct Support Providers to work one-on-one with children and adults with developmental disabilities in their homes and communities. Our providers play an important role in helping members build independence, increase safety, and improve quality of life.

Services Provided:

Habilitation (HAB)

· communication and social skills

· behavior support and coping strategies

· personal hygiene and self-care routines

· community safety and participation

· meal preparation and household routines

· transitions and daily schedules

· age-appropriate developmental skills

Attendant Care (ATC)

· bathing / hygiene assistance

· dressing and grooming

· toileting and incontinence support

· feeding / meal assistance

· mobility and transfers

· supervision for health and safety

· light support related to the member’s care needs

Respite

· supervision and engagement

· assistance with routines

· community outings

· play and recreational activities

· emotional support and companionship

Position Responsibilities

· Provide safe, respectful, and reliable care in the member’s home and community

· Follow the member’s ISP/service plan and family preferences

· Maintain professionalism and confidentiality

· Communicate clearly with TEAM supervisors and families

· Arrive on time and maintain consistent attendance

· Complete required documentation accurately and on time

· For habilitation:complete daily progress / SOAP notes related to goals worked on

· For attendant care: complete daily service logs / task documentation

Required Qualifications

· Must be at least 18 years old

· High school diploma or equivalent

· Reliable transportation and valid driver’s license (if transporting members)

· Ability to pass required background checks

· Dependable, patient, and compassionate

· Able to work independently and follow directions

Required Training / Compliance

· Arizona Article 9 training

· CPR / First Aid certification

· Fingerprint clearance / background check requirements

· Any DDD / DES required onboarding and compliance documents

· TEAM orientation and ongoing training as needed

Pay Range

· Starting pay: $16.00 – $19.00 per hour, depending on experience, availability, and assignment

· Higher rates may be considered based on qualifications, experience, and specific member needs
