A Tennessee-based health services network is seeking a Health Navigator to support members through benefits education, care gap outreach, and coordination of health-related resources.
About the Opportunity:
- Schedule: Monday through Friday
- Hours: Standard business
- Setting: Hybrid (3 days onsite)
Responsibilities:
- Support the clinical team by serving as a resource for members related to disease prevention and benefit education
- Conduct educational phone calls to explain available benefits, services, and programs
- Complete health needs assessments and refer members to appropriate population health management programs
- Contact members with care gaps, encourage compliance, and assist with provider search and appointment scheduling
- Manage work queues, review member information, and route cases for outreach and intervention
Qualifications:
- At least 2 years of experience in a Customer Service support role
- Associate's degree in Education, Communication, Health, or a related field, or equivalent work experience
- Knowledge of Medical terminology
- Strong interpersonal, organizational, communication, time management, and problem-solving skills
- Ability to work independently, manage multiple tasks, and set priorities
Desired Skills:
- Experience working in a Healthcare or Health Insurance environment.
- Familiarity with system work queues, Claims Review, Eligibility Screening, or Authorization History
- Experience with Member Outreach, Care Coordination, or Health Education
Salary : $14 - $16
