About the Role
As our Admissions & Discharge Planner, you’re the person who makes sure no one falls through the cracks. You’ll welcome clients on their very first day, coordinate the appointments and transportation that keep their care on track, and begin planning for a successful discharge from the moment they arrive. You’ll connect clients to housing, healthcare, employment, and community resources — and keep the whole care team moving in sync. It’s a role with real ownership and real human impact, perfect for someone who loves building systems, solving problems, and seeing the difference their work makes in people’s lives.
What You’ll Do
- Welcome new clients. Lead intake and orientation, walk clients through their admission paperwork, and make sure every signature and document is complete and filed within 24 hours.
- Keep care on track. Schedule and confirm Nurse Practitioner appointments, arrange Medicaid or alternative transportation, and confirm with clients in advance so no appointment is ever missed.
- Plan for success from day one. Begin discharge planning at admission, maintain a discharge plan for every active client, and launch referrals for housing, employment, food, healthcare, and behavioral health support as discharge approaches.
- Connect clients to community. Link clients to housing, healthcare, employment, and social services, help them navigate complex systems, and follow up within 48 hours to confirm every referral actually lands.
- Champion accuracy and compliance. Submit documentation within 24 hours, keep the ClickUp Admissions/Discharge Tracker current by Friday at 5:00 PM, and protect the accuracy and confidentiality of every record in line with Virginia Medicaid and DBHDS standards.
- Be part of the team. Join treatment team meetings, watch for and report relapse indicators, support outreach that keeps clients engaged, and step in to help cover counseling or administrative needs when the team is short-staffed.
What You’ll Bring
- An associate’s degree in human services or a related field, plus at least one year working with individuals experiencing mental illness, substance use disorders, or developmental disabilities — or one year of addiction/recovery experience with a CSAC-A credential.
- A valid Virginia driver’s license and a current DMV driving record (you’ll transport and link clients to appointments).
- CPR, First Aid, and Behavior Modification certifications, or the willingness to obtain them.
- A plus: LPC, LPC-Eligible, LCSW, LCSW-Eligible, CSAC, CSAC-S, or CSAC-A licensure or credential.

$22.00-$26.00 per hour depending on education and experience.
