What are the responsibilities and job description for the Residential Crisis LMHP/LMHP-R position at TANDEM COUNSELING & CONSULTING LLC?
About the Role
As our Licensed Mental Health Professional, you’ll provide clinical leadership and hands-on therapeutic care in a DBHDS-licensed Residential Crisis Stabilization Unit (RCSU) serving adults experiencing acute behavioral health crises. This is a 12 bed male/female facility. This is a role for a clinician who is equally comfortable across the desk from a client in distress and across the table from a treatment team that looks to them for direction.
You’ll co-own the clinical picture from admission to discharge with the Nursing Supervisor as your counterpart. You will be conducting assessments, building individualized service plans, facilitating or Co-facilitating therapy, guiding unlicensed staff, and making sure every service we provide is trauma-informed, recovery-oriented, and fully compliant with Virginia Medicaid and DBHDS standards.
Why You’ll Want This Job
- Real impact, every shift. You’ll see the difference your work makes, not in a quarterly report, but in the person who walks out stabilized and connected to ongoing care.
- Clinical autonomy with support. You’ll be the non-medical clinical lead on shift, trusted to make the call, backed by an interdisciplinary team and a Clinical Director who has your back.
- Growth that’s built in. LMHP-R? We support clinicians working toward full licensure. Looking to lead? Supervisory experience is valued and developed here.
- A team that shows up. You’ll work alongside peer recovery specialists, QMHPs, case managers, and medical staff who pull in the same direction.
-
Clinical Care & Crisis Intervention
- Conduct crisis assessments at admission to determine level of care, diagnoses, and individualized needs.
- Facilitate crisis intervention services, including de-escalation and risk/safety assessments (e.g., suicide risk, aggression).
- Deliver individual, group, and family therapy focused on stabilization, coping skills, and crisis resolution.
- Serve as the non-medical clinical lead on shift and on call — reviewing client status and addressing urgent concerns as they arise.
-
Treatment Planning & Care Coordination
- Develop and oversee Treatment Plans / Individualized Service Plans (ISPs) within required timelines and in line with DMAS/DBHDS standards.
- Complete Initial and Continued Stay Authorization requests and provide additional documentation as requested.
- Collaborate with medical staff, peer recovery specialists, case managers, and other interdisciplinary team members.
- Participate in team meetings and contribute to ongoing treatment planning and discharge readiness reviews.
- Coordinate discharge planning with outpatient providers, case managers, family, and other supports to ensure continuity of care.
-
Clinical Oversight & Supervision
- Provide clinical oversight of care delivered by QMHPs and other unlicensed staff.
- Supervise, coach, and support designated staff and residents under your care.
- Ensure services remain trauma-informed, recovery-oriented, and person-centered at every step.
-
Documentation & Compliance
- Maintain accurate, timely documentation assessments, progress notes, and ISP updates
- Maintain compliance with DBHDS, the Office of Human Rights, OSHA, and all applicable federal, state, and local regulations.
- Report incidents of low engagement, behavioral concerns, or suspected abuse/neglect to the appropriate personnel.
