The RN – MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments for residents in a long-term care setting. The coordinator works closely with interdisciplinary teams to maintain quality outcomes and resident-centered care.

Requirements
- Active RN license (Required)
- Experience in LTC / Skilled Nursing Facility preferred
- MDS coordination or MDS coding experience preferred
- Strong understanding of CMS regulations and care planning processes
- Ability to complete accurate and timely resident assessments
- Strong documentation and communication skills
- Familiarity with EPIC charting system preferred
- Active Registered Nurse (RN) License
- BLS Certification (Preferred depending on facility policy)
- 50-bed Skilled Nursing Facility
- EPIC EMR System
- Break: 30 minutes per shift
- Scrub Policy: Any scrubs allowed except all-black sets (top and bottom together not permitted)
- Long-Term Care / Skilled Nursing Facility
- Interdisciplinary care team environment
- Focus on resident assessments, care planning, and regulatory compliance
