Education & Experience
- Associate’s or Bachelor’s Degree in Nursing required.
- 1–2 years of home health, home care, medical-surgical, or community nursing experience preferred.
- Experience providing independent patient care in a home setting is preferred.
- Active Registered Nurse (RN) license required.
- BLS certification required.
- Additional home health or specialty certifications preferred based on patient needs.
- Perform comprehensive assessments of patients in the home and identify changes in condition.
- Develop, implement, and evaluate individualized nursing care plans.
- Administer medications, wound care, IV therapy, and other prescribed treatments.
- Monitor patient progress and identify potential complications requiring medical intervention.
- Educate patients and families on medications, treatments, disease management, and safety.
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
- Maintain accurate clinical documentation and follow infection control, patient safety, and home care standards.
