Responsibilities
- Provide high-quality physical therapy services to patients on-site.
- Assess patient needs and develop individualized treatment plans.
- Meet or exceed weekly/monthly productivity and outcomes targets.
- Record progress and outcomes in patient records with accuracy.
- Monitor patient response and adjust treatments accordingly.
- Utilize data and pipelines to identify potential patient referrals.
- Collaborate with physicians and teammates to coordinate care.
- Educate patients and caregivers on home exercises and self-management.

- Experience using data pipelines or analytics to identify referrals
- Patient and caregiver education on home programs and self-management
- Maintain compliant clinical environment and equipment, including infection control
- Strong communication with leadership; reporting progress and obstacles timely
