Responsibilities
Provide direct patient care and support under the supervision of registered nurses.
Administer medications and monitor patient responses to treatments.
Assist in developing and implementing patient care plans.
Participate in ongoing professional development and training opportunities.
Maintain accurate patient records and documentation.
Collaborate with healthcare team members to optimize patient outcomes.
Engage in performance measurement and strive to meet individual targets.
Monitor vital signs and report any changes to the nursing staff.
Educate patients and families on health management and care procedures.
Ensure compliance with healthcare regulations and standards.
Assist with daily living activities for patients as needed.
Experience/Qualifications
Valid LPN/LVN license and current CPR certification.
Commitment to ongoing professional development and training.
Experience in direct patient care in a clinical setting.
Ability to monitor vital signs and report changes accurately.
Willingness to engage in performance measurement and target achievement.
Strong communication skills for patient and team interactions.
Ability to work independently and within a team environment.
Understanding of healthcare regulations and compliance standards.
Compassionate approach to patient care and support.
Familiarity with electronic health records (EHR) systems.
