Responsibilities
- Provide direct patient care under the supervision of registered nurses.
- Administer medications and monitor patient responses.
- Participate in ongoing professional development and training.
- Assist in developing and implementing patient care plans.
- Collaborate with healthcare team members to optimize patient outcomes.
- Engage in performance measurement and improvement initiatives.
- Maintain accurate patient records and documentation.
- Educate patients and families on health management and care procedures.
- Ensure compliance with healthcare regulations and standards.
- Monitor vital signs and report changes to the healthcare team.
- Support a positive work environment through teamwork and accountability.
- Assist with daily living activities for patients as needed.
- Valid LPN license in the state of practice.
- Commitment to ongoing professional development and training opportunities.
- Experience in a healthcare setting, preferably in direct patient care.
- Strong understanding of medication administration and patient monitoring.
- Ability to collaborate effectively with a multidisciplinary healthcare team.
- Excellent documentation skills for maintaining accurate patient records.
- Ability to monitor vital signs and recognize changes in patient conditions.
- Knowledge of healthcare regulations and compliance standards.
- Strong interpersonal skills to educate patients and families effectively.
- Willingness to support daily living activities for patients as needed.
