Duties
- Coordinate care for patients in the Emergency Department and manage responsibilities in the Transfer Center as a Clinical Admission Coordinator (CAC).
- Perform care coordination, concurrent reviews, continued stay reviews, and discharge planning.
- Determine medical necessity per evidence-based guidelines.
- Conduct needs assessments and order Durable Medical Equipment (DME) as needed.
- Develop and manage patient plans of care.
- Utilize EPIC documentation system and SBAR communication system.

- Basic Life Support (BLS) certification required
