Duties
- Coordinate and manage patient care, ensuring efficient discharge planning and care coordination.
- Organize and participate in family meetings to develop comprehensive treatment plans.
- Assess patients and facilitate the arrangement of DME, SNF, and ambulance transport independently.
- Work with HMOs, IPAs, and other healthcare organizations to manage patient needs.
- Provide care for patients with diverse needs, including those with mental health histories, experiencing homelessness, or requiring protective services.
- Ensure compliance with CMS, Department of Health, HIPAA, OSHA, and The Joint Commission standards.
- Utilize computerized documentation systems, specifically Epic, to maintain accurate patient records.

- Appeals and Denials
- Care coordination

- Needs Assessment/Order DME
- Plan of Care
- Worker’s comp case management
- Regulatory knowledge including CMS, Department of Health, DRG, HIPAA, OSHA, The Joint Commission
- Experience in Emergency Department*, ICU*, Surgical Services*
- Minimum of 2 years of acute medical Care Manager experience performing Care Coordination.
- Experience in Trauma Level I and II settings is preferred.
- Travel experience is required.
- Experience in a teaching hospital, community hospital, or LTAC is beneficial.
- Experience with Epic charting system is required.
Salary : $65 - $70
