Duties
- Provide comprehensive case management services, including care coordination and discharge planning.
- Organize and participate in family meetings and treatment plan development.
- Arrange DME, SNF, and ambulance transport independently.
- Utilize computerized documentation and the Epic charting system.
- Collaborate with interdisciplinary teams, including IV Teams, Physical Therapy, Respiratory Services, and more.
- Participate in the development of treatment plans and special procedures.

- Experience with a patient ratio of 1:20.
- Epic charting system experience required.
- Preferred experience in teaching hospitals, community hospitals, LTAC, Trauma Level I and II facilities.
- Travel experience required.
- BLS certification required.
