Pharmacist

Job Summary

Under the supervision of the Clinical Coordinator or Manager and the Pharmacy Director, the Clinical Pharmacist optimizes patient medication therapy by providing comprehensive clinical pharmacy services. This role works collaboratively with the healthcare team to support patient care goals and ensure safe, effective medication management.

Job Requirements

Minimum Education

• B.S. in Pharmacy, PharmD, or Master of Pharmacy
• Completion of continuing education as required by the Texas State Board of Pharmacy (TSBP)
  
  

Minimum Work Experience

• Previous hospital experience with clinical responsibilities
  
  

Required Licenses/Certifications

• Active pharmacist license in the state of Texas
• Sterile Compounding Certification
• BLS, ACLS, or PALS (based on practice area)
• TSBP Preceptor registration
  
  

Required Skills, Knowledge, And Abilities

• Ability to assess, document, and perform physician consultations, including all medication dosing consults (excluding total parenteral nutrition)
• Proficiency in performing home medication reconciliation
• Ability to provide educational presentations to pharmacy staff and other departments as needed
• Strong leadership and supervisory skills
• Ability to handle confidential information with discretion
• Professional appearance and demeanor at all times
• Excellent oral and written communication skills
