Summary:
- Job Summary: Coordinate and provide restorative and rehabilitative Occupational Therapy services to participants at the Center and at home.
- Work closely with PACE Physician, rehabilitation staff, and other interdisciplinary team (IDT) members.
- Collaborate with the Physical Therapist to efficiently operate Client rehabilitation department.
- Facilitate collaborative relationships across Client and with external stakeholders.
- Conduct Occupational Therapy assessments to determine care needs, preferences, and goals of participants.
- Deliver and document Occupational Therapy interventions in accordance with physician orders and care plans.
- Supervise Therapy Assistants, providing training and guidance needed to implement Functional Maintenance Programs (FMP).
- Provide training and instructions to staff, participants, and caregivers/families in areas of Occupational Therapy.
- Order, track, and manage maintenance for equipment.
- Update the IDT promptly of any changes in participants’ condition or functional status.
- Support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home).
- Support enrollment of prospective participants into the program.
- Participate in end-of-life care coordination and support.
- Manage direct reports including training, supervising, and mentoring.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information.
- Follow Client policies and procedures and participate in Quality Improvement activities.
- Communicate regularly with Supervisor and team regarding workload and priorities.
- Timely completion of all mandated trainings and education.
- Exercises flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
- Required Skills & Experience:
- Minimum of two years Occupational Therapy experience in a clinical setting with a frail or elderly population.
- Ability to innovatively engage, support, and remove barriers for the team to ensure goals are met.
- Occupational Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.
- Two years’ supervisory experience with demonstrated ability to mentor and develop team members.
- Excellent leadership, organizational, and communication skills.
- Experience leading in a data-driven organization, leveraging reports and data.
- Ability to work independently with minimal supervision.
- Experience and competency working with people from diverse backgrounds and cultures.
- Commitment to unlocking the full potential of our most vulnerable seniors.
- Required Education: Bachelor’s Degree in Occupational Therapy from Accredited Program (AOTA and ACOTE standards).
- Required Certifications & Licensure: Initial Registration from National Board for Certification in Occupational Therapy. Licensed and/or eligible for Licensure as required in the State of California. BLS.
