Job Summary
The Home Care Assistant is responsible for providing safe delivery of personal care services to participants in their homes, as scheduled and directed. Collaborate effectively with colleagues and stakeholders to promote team values, culture, and mission.
Job Responsibilities
- Provide personal care and assistance with activities of daily living (ADLs) to participants in their homes, including but not limited to bathing, dressing, grooming, toileting, and transferring.
- Assist participants with Instrumental Activities of Daily Living (IADLs), including but not limited to laundry, light housekeeping, and shopping.
- Assist with light meal preparation and feeding in participants' homes, ensuring participants are nurtured and hydrated.
- Assist participants with safe transfer in and out of their homes and act as escort on vehicles and to medical appointments as assigned.
- Maintain personal care supplies such as incontinent supplies and personal grooming items in participants' homes.
- Assist Rehabilitation department by carrying out a home exercise program as indicated in the care plan developed by the IDT, after sufficient training.
- Remind participants of medications.
- Provide translation services as needed, if qualified.
- Remain alert and update the supervisor or any member of the IDT promptly of any changes in participants' condition.
- Must be willing and have the ability to work a varied schedule that may include evenings, nights, weekends, and overtime.
- Be legally authorized to perform in position location, required.
- Only act within the scope of his or her authority to perform.
- Complete all required documentation in a timely and accurate manner.
- Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
- Follow policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
- Communicate regularly with Supervisor and team regarding workload and priorities.
- Timely completion of all mandated training and education.
- Timely completion of all mandated occupational health screenings as needed.
- Exercises flexibility in performing assignments as business needs evolve.
- Other duties as assigned.
Required Skills and Experience
- Minimum of one (1) year of experience providing personal care to a frail or elderly population.
- Ability to follow directions from supervisor and communicate needs effectively.
- Excellent organizational and communication skills.
- Ability to work independently in participants' homes, without direct supervision.
- Demonstrated ability to prioritize in a fast-paced environment.
- Experience and competence working with people from diverse backgrounds and cultures.
- Commitment to unlocking the full potential of our most vulnerable seniors.
Required Education
- High School Diploma or Equivalency.
- OR Professional experience of one (1) year may be substituted.
Required Certifications & Licensure
- BLS
- Must be registered on the California State Home Care Registry (This is a Client managed item).
